Question

27 year old female reported for crown lengthening procedure-14 . On examination, a buccal growth seen in region of 14 with adequate keratinized gingiva. What is the treatment?

Gingivectomy

•            The word gingivectomy means “excision of the gingiva.”

•            By removing the pocket wall, gingivectomy provides visibility and accessibility for complete calculus removal and thorough root planing.

•            This creates a favorable environment for gingival healing and restoration of a physiologic gingival contour.

•            Gingivectomy may be performed for the following indications :

1.           Elimination of suprabony pockets if the pocket wall is fibrous and firm

2.           Elimination of gingival enlargements

3.           Adequate keratinized tissue

•            Contraindications to gingivectomy include the following:

1.           Access to bone required

2.           Narrow zone of keratinized tissue

3.           Aesthetics

4.           Patients with high postoperative risk of bleeding

 

 

Key concept:

•            One of the most important requirements of gingivectomy is the presence of adequate attached gingiva.

•            This is because, after the surgical procedure is complete, the tissues shrink on healing. In cases where there is inadequate attached gingiva, the treatment plan should focus on reattachment procedures which would help increase the width of the attached gingiva.

•            One of the most important functions of the attached gingiva is the transfer of the occlusal stresses.

•            Thus, all attempts have to be made to maintain or increase the width of the attached gingiva.

•            In the given question, it has been mentioned that there is adequate attached gingival width. Thus, post a gingivectomy, there would not be any decrease in the width and is therefore the answer of choice in this question.
